Initial Clinical Consultation
Face-to-face or remote consultation with a nurse / advanced nurse practitioner
Includes:
- Weight, height, BMI, waist measurement (if appropriate)
- Blood pressure and relevant observations
- Assessment of obesity-related health risks
- Discussion of goals, motivations, and readiness for change
- Education provided on health risks and treatment options
Individualised Treatment Plan
Shared decision-making with the patient
Plan may include:
- Nutrition and lifestyle advice
- Behavioural strategies
- Blood tests (if clinically indicated)
- Prescription weight-management medication (subject to eligibility and clinical assessment)
- Risks, benefits, side effects, and expectations clearly explained
- Informed consent documented
Initiation of Treatment
Treatment commenced where appropriate
Patient provided with:
- Written advice and treatment plan
- Information on medication use and side effects (if prescribed)
- Safety-netting advice and contact details
Ongoing Monitoring & Follow-Up
Regular follow-up appointments (e.g. 4–12 weekly)
Review of:
- Weight and measurements
- Tolerance and effectiveness of treatment

- Adjustments made to treatment plan as required

Maintenance & Long-Term Support

- Weight maintenance

- Gradual reduction or cessation of medication where appropriate
- Discharge with self-management advice or continued periodic reviews
Escalation, Referral & Discharge
- Referral to GP or specialist services if required
- Clear documentation and communication (with consent)
- Patient discharged when goals achieved or service no longer required
